Forms
The forms indicated below are needed by all coaches to be used when applicable. No other forms may be substituted. Upon completion of the form, please submit to Paul Schmidt as soon as possible.
Waiver form is required immediately upon selection / inclusion of players on Team.

Use of this dental form is expected for any injury that happens while the player is involved with the Team.
Include is the proof of insurance that may be required.
For any medical injury, it is best to print out copies of the Medical form so that the Team Manager has on hand and may be completed immediately at the hospital or medical facility. It MUST be completed totally, all sections prior to submission, which is required to be done immediately upon completion, which normally would be within two days of the injury. It can be hand delivered or faxed to Paul Schmidt.
The Waiver MUST be completed as soon as possible, the Players names, Players signature and Parents signature and submitted.
